What was the story behind the album’s iconic cover art?

The cover photograph, shot by Kirk Weddle, features a naked baby boy swimming underwater toward a dollar bill on a fishhook. The baby was Spencer Elden, the three-month-old son of a friend of the art director. The image was conceived by Kurt Cobain after he watched a documentary about underwater births.
